Since your response to the survey is anonymous, it is not possible to submit a complaint to the Ombudsman in the survey. Respond as soon as possible. The survey will be collected from the prison on 14 April 2020. If you need more space, please feel free to write on the other side of the paper. When you are done, put the survey in the envelope addressed to the Ombudsman (JO) . Seal the envelope. Give it to a staff member of the Prison and Probation Service. There is no need for a stamp.
